The percentage of perfect, mistake-free hands in Texas Hold'em is infinitesimally small. There are 2.6 million possible starting hands, yet countless ways each hand can play out. The countless betting sequences, calls, folds and mucks that can ensue mean even a perfect initial hand may yield an imperfect result. While a skilled player improves their odds, an element of luck remains. A hand well-played is not one without error, but one that minimizes regret. Perfection is unattainable at the poker table.
